Case Manager - Assisted Living Program

Utica Senior Living  is actively seeking a full time Case Manager for our Assisted Living Facility in Utica, NY.



  • Marketing when necessary

  • Licensed social worker recommended

  • Assist in orienting new residents to the community.  This might include ensuring appropriate paperwork is received, alert interdisciplinary team about admission, prepare physical chart, check apartment readiness, provide keys, take resident’s picture for medical record, review advance directives, alert Medicaid and Social Security as needed, seat resident in the dining room.

  • Input all resident information into computer software system prior to arrival and update as information changes.
  • Complete case notes in a timely manner as appropriate based on department of health regulations & guidelines. (Initial, 30day, annual—interim notes for hospitalization/weight changes/medical issues)
  • Assist in maintaining or obtaining SSI/SSD benefits (phone calls setting up appointment, obtain information for appointment with resident/resident’s family, assist resident on day of appointment to help resident as needed.)
  • Work with residents to complete Access-A-Ride application and associated tasks
  •  Coordinate medical appointments as needed (including but not limited to coordinating transportation with MAS, solving eligibility issues, coordination with MD office, scheduling visiting dental, PT/OT,  podiatrist, optometrist/ophthalmologist, alerting residents of appointments)
  • Facilitate readmissions from hospitalization or rehabilitation by reviewing paperwork for readmission, completing case notes, contact family members, work with interdisciplinary team and other agencies to ensure proper services are in place
  • Incident reporting (maintain incident reports, notes, follow-up, binder) 
  • Order durable medical equipment based on physician orders
  • Collaborate with finance office to assist with Medicaid recertification or SSA concerns
  • Assist in Resident concerns which might include: complaints, roommate issues, room changes, behavioral problems, various interventions, dining room seat changes
  • Monthly weights
  • Ensure completion of Mental Health evaluations and coordination psychologist or psychiatrist services as needed.
  • Work with residents to identify their needs that are not met and seek out resources or      alternate placement as indicated.
  • Strengthen the informal relationships among residents.
  • Conduct resident assessments and participate in marketing efforts as needed.
  • Work with other resources to coordinate resident services located inside or outside the home. These may be provided by formal or informal helpers (i.e. staff, professionals, family members or volunteers)
  • Support the various ethnic and cultural groups that reside in the facility.
  • Assist the Administrator as a representative of the Home in the Community.
  • Act as a resident spokesperson when necessary.
  • Review resident’s service needs periodically.
  • Assist residents in participating in planning for changes or improvements in the home.


  • Must be at least 21 years of age.
  • Must have a baccalaureate degree in human services, social services, clinical services, health service management, business administration and/or other pertinent fields of study;
  • 3 + years work experience preferred.
  • Must be able to communicate effectively utilizing oral, written, and technical modes of communication.
  • Fluency in the Spanish language preferred.
  • Must be computer literate and capable of utilizing word processing and spreadsheet programs.
  • Must be a professional representative of the organization to the community, regulatory bodies, and any other outside entities.
  • Must be able to lead by example as demonstrated by consistently timeliness, task delegation, constant follow up, offering direction, and always available to clients and employees alike in a supporting role.
  • Must be able to assess the necessary steps to take to resolve dilemmas which may be outside the scope of regulatory, company, or industry standards.
  • Must be very organized and able to complete standard office tasks with minimal assistance.
  • Must demonstrate sound judgment when making decisions, considering client rights, internal and external politics, and existing hierarchies.
